I, the undersigned, being the parent or legal guardian of the above-named child ("Participant"), hereby acknowledge and agree to the following terms and conditions: 1. Assumption of Risk I understand that participation in games, dodgeball, video games, and other activities involves inherent risks, including but not limited to physical injury, falls, collisions, and other unforeseen incidents. I voluntarily assume all such risks associated with my child's participation in this event. 2. Release of Liability In consideration of allowing my child to participate, I hereby release, waive, and parent night out, its owners, directors, officers, employees, volunteers, and agents from any and all claims, liabilities, demands, actions, or causes of action arising out of or related to any injury, loss, or damage that may occur as a result of participation in the event. 3. Medical Treatment Authorization In the event of a medical emergency, I authorize the event staff to obtain necessary medical treatment for my child. I understand that I am responsible for any medical expenses incurred as a result of such treatment. 4. Photo and Media Release I grant permission for my child’s image to be used in photographs or videos taken during the event for promotional purposes. I understand that no compensation will be provided for such use. 5. Acknowledgment of Event Policies I agree to abide by all event rules and guidelines set by the organizers, including but not limited to behavior expectations and safety protocols. I understand that failure to comply may result in my child's removal from the event without a refund. I HAVE READ AND UNDERSTAND THIS WAIVER AND RELEASE OF LIABILITY.
